Default Hole in piston

I have a 94 Monte Carlo Tigershark with a blown rear cylinder. There was a hole in the piston. I replaced the piston and rings and all gaskets, within 3 hours I had burnt a hole in the new piston. Front cylinder is fine. After close inspection the only thing I could find was that the reed valve for that cylinder wasn't fully seated. Could this be my problem or is there something else that is causing the holes in the piston. Any ideas?

If it was letting more air in, then the cylinder ran lean which could be a possible cause for the hole in the piston.

Thanks, I'm in the process of changing the front and rear seals, hoping that solves the problem...
